Cytokinetics posts positive ACACIA-HCM data for aficamten, and the stock falls 6.2%

CYTK said its myosin inhibitor aficamten, branded Myqorzo, delivered efficacy and a manageable safety profile in patients with non-obstructive hypertrophic cardiomyopathy in the ACACIA-HCM trial. The data were presented at the European Society of Cardiology Congress 2026 and published in the New England Journal of Medicine. The company framed the readout as a step toward broader cardiomyopathy indications.

The commercial significance is the label rather than the trial. Aficamten's current path is anchored in obstructive disease, and non-obstructive HCM is a distinct and larger patient group with far fewer targeted options. Cytokinetics has signalled an FDA submission for the expanded indication, and a peer-reviewed publication alongside a major congress presentation is the kind of evidence package that supports one.

The tape is not celebrating. SentiSense has Cytokinetics at $73.10, down 6.2%, even though its score warmed to 25.4 against a 30-day average of 4.9 and latest sentiment reached +0.60. That divergence between a positive news reaction and a negative price reaction usually means the data were already anticipated, or that the read-through to timelines was less aggressive than hoped.

Options positioning points the other way. The put/call volume ratio has fallen to 0.02, the 4th percentile of its one-year range, meaning call activity is unusually dominant for this stock. What to watch is the submission timeline itself, any advisory committee signal, and whether the label expansion arrives broad enough to reprice the addressable population rather than just confirm the existing one.
