Elevance Health Beats Q3 Estimates and Lifts 2026 EPS Guidance to at Least $29, Though Most of the Beat Was Non-Operating

Elevance Health reported third-quarter adjusted EPS of $6.99, far above the roughly $4.70 consensus, on operating revenue of $50.44 billion, up 0.7% from a year earlier. About $1.85 per share of the beat came from non-operating items. The insurer raised its 2026 adjusted EPS guidance to at least $29 from at least $27, and its benefit expense ratio improved to 90.6%. Shares swung from a roughly 4% intraday drop to about flat by early afternoon.
ELV reported third-quarter 2026 adjusted earnings of $6.99 per share on October 21, well above the $4.71 consensus cited by MarketBeat, on revenue of $50.44 billion. GAAP diluted EPS was $6.27, up from $5.32 a year earlier, and adjusted EPS rose from $6.03, according to the company's release. Operating revenue grew just 0.7% year over year, so the beat came from profitability rather than top-line growth.
The quality of the beat matters more than its size. BigGo's breakdown of the results attributes about $0.50 per share to operating outperformance and $1.85 per share to a non-operating benefit, a split Investing.com also reported. On the operating side, the benefit expense ratio improved 70 basis points to 90.6% from 91.3%, which the company attributed to better performance in Medicare Advantage and Individual ACA. Carelon operating gain rose 17% to $0.9 billion on $19.3 billion of operating revenue, while total medical membership fell 1.1% from a year earlier.
Elevance raised its 2026 adjusted EPS outlook to at least $29 from at least $27 and set GAAP guidance at no less than $22, with operating cash flow of at least $6.5 billion. It also targeted at least 12% adjusted EPS growth in 2027 from what it called a higher earnings baseline of at least $26.75. Some coverage described that as a $0.75 raise to the 2027 baseline, but the company's release states no prior baseline figure: the $26.75 is the adjusted 2026 base that the 2027 growth target is measured from, excluding non-recurring investment income and one-time items.
Shares took a volatile path. The stock opened at $446.25 and was down 4.38% at $426.70 by 10:20 a.m. ET, according to a 24/7 Wall St. price card, before recovering to $440.86, up 0.2% from the prior $440.18 close, at 1:26 p.m. ET in SentiSense price data. Analysts lean positive: SentiSense consensus data shows 14 buy and 8 hold ratings and an average target of $451.71, while Barclays raised its target to $469 on October 7. The next questions are whether medical cost trends in Medicare Advantage and ACA stay favorable into 2027 and how much of this quarter's earnings power repeats once non-operating items fall away.
