Novartis Ends Mid-Stage Fabhalta Trial in Lupus Nephritis, Citing Strategic Reasons

Novartis terminated its Phase 2 trial of Fabhalta (iptacopan) in lupus nephritis after completing the study's first part, citing strategic considerations rather than any disclosed safety or efficacy finding. Fabhalta remains approved for PNH, IgA nephropathy and C3 glomerulopathy.
NVS has ended a mid-stage trial of Fabhalta in lupus nephritis, a serious kidney complication of lupus, Reuters reported. The ClinicalTrials.gov entry for the study, NCT05268289, was updated on October 15 to show it as terminated, with the reason given as a "decision to conclude the study after Part 1 LPLV is based on strategic considerations by the Sponsor." LPLV refers to the last patient's last visit, so the first part of the study ran to completion before Novartis stopped.
The trial was a Phase 2 adaptive, randomized, double-blind, placebo-controlled dose-exploration study of iptacopan in patients with active lupus nephritis. Neither the registry nor the coverage reviewed gave a safety or efficacy reason for ending it, and Novartis has not published results from the completed portion, so the decision should not be read as a failed readout on the available evidence.
Fabhalta, an oral complement factor B inhibitor, is already approved for paroxysmal nocturnal hemoglobinuria, IgA nephropathy and C3 glomerulopathy, and those franchises are unaffected by this decision. Lupus nephritis had been one of the drug's investigational expansion indications, so the termination narrows the long-term label opportunity rather than touching current sales.
What to watch: whether Novartis discusses the decision or the Part 1 data on its next earnings call, whether other Fabhalta expansion programs continue on schedule, and how the move reshapes competition in lupus nephritis, where other companies are developing complement-targeting therapies.
