Novo Nordisk's Semaglutide Trial Shows Majority of Young Participants Drop Below Obesity Threshold

NVO reported results from STEP Young, a trial of semaglutide in children aged 6 to under 12 with obesity, in which 40.4% of treated participants fell below the obesity threshold at week 68 against 0% on placebo . The figure carries an important qualifier that most secondary coverage drops: it is a trial-product estimand, meaning it describes the effect assuming full adherence to treatment rather than the plain observed result across everyone enrolled.

The population makes the result more striking than the headline percentage alone. More than 85% of participants entered the trial with Class II or Class III obesity, so the cohort was severe rather than borderline, and the study enrolled 165 children dosed on a weight-based schedule to 1.7mg or 2.4mg weekly.

Safety is the variable that determines whether this becomes a commercial product in this age group. Novo reported no signal in growth or pubertal development, which is the specific concern regulators and paediatricians raise about GLP-1 use in children . A single 68-week trial does not settle a question about developmental effects, but the absence of an early signal is the necessary first condition.

Full data are set for presentation at ObesityWeek on November 14 to 17. What to watch: whether the observed-population numbers, once disclosed alongside the estimand, hold close to 40%; how regulators weigh a paediatric label; and whether payers extend coverage to a group where the treatment duration could run for decades. That last question is the one that determines the commercial size of the opportunity, and it is not answered by efficacy data.
